Care opened it’s doors May, 2005 and continues to focus daily on living up to it’s reputation of being the area’s leading patient centered, family practice clinic. We are dedicated to providing front-line integrated medical care with constant attention to the patient-provider relationship. Our policy is to initiate care for individuals needing health care regardless of age, gender, race, ethnicity of socioeconomic status. We are constant advocates for our patients and community.

Care is considered by most of it’s patients as their personal “medical home”. Our providers deliver a range of acute, chronic and preventive care, including routine check ups, health-risk assessments, immunizations and screening tests, often including personalized counseling on healthy lifestyle maintenance. We also assist in managing chronic illness and often coordinate care with subspecialists.

Care is involved in the primary and continuum care of our community and it’s health care needs. Strong foundational precepts such as:
